You must submit a completed Declaration of RTS Funding Arrangements, one copy of your birth certificate, photo page from passport, humanitarian visa or evidence of permanent residency status, and if applicable, proof of withdrawal from previous study, a list of publications, names of academic referees, and evidence of clinical registration.
